While in the residential program, the individual and their case manager begin planning a comprehensive strategy for a long-term recovery. The goal of transitional care is to provide clients with a semi-structured, sobriety-valued environment that empowers individuals to continue their long-term recovery process when they transition back to their normal environments.

In the transitional care program, individuals will focus on their personal and professional goals and will begin to implement them in their everyday lives. Focused on restoring the mind, body, and soul when integrated back into normal life, the transitional program encompasses a whole person approach to long-term addiction recovery.
Half-day courses provide a more comprehensive understanding of their diseases and a deeper working relationship to the principles and spirituality found in the 12-Steps. Understanding that the recovering addict must essentially re-shape much of their former lifestyle, healthy goals are set, “tools” for living a sober life are provided, and new patterns for relapse prevention are set into place.
